Annual Wellness Visits – Frequently Asked Questions

Annual Wellness Visits, also known as AWVs, are yearly visits for Medicare patients to create or update a personalized prevention plan by gathering information through the use of a Health Risk Assessment questionnaire. 80% of Medicare-eligible patients do not complete these each year. AppleCare has decided to make a concerted effort to increase our involvement in looking at prevention as part of the care we deliver. Early intervention and prevention help save lives. This is why we at AppleCare have started asking our eligible members to complete these questionnaires and encouraging our patients to follow up with their primary care providers.
